Abstract

Despite the many years during which the syphilis has been subjected to intense scientific scrutinity, both its natural history and the optimal therapeutic approach continue to defy precise description. More, in the last years, the HIV infection/AIDS has, to a certain extent, undermined the assumed validity of models of syphilis staging and of the understanding of the clinical and serological response to therapy. There are a series of questions that address the central and often ambiguous area of syphilis management: the dose of 2,4 million UI banzathine penicillin, the alternatives to benzathine penicillin, the adequate therapy of neurosyphilis, the approapriate therapy for the patients with syphilis and HIV infection, penicillin allergy, the criteria for the cure of syphilis and persistent infection.
