Abstract
This article reviews epidemiology, clinical manifestations (major manifestations, minor manifestations and documentations of an antecedent streptococal pharyngitis), pathogenesis tratament and prophylactic therapy of acute fever. A description is given of the principal mutations in the natural evolution of the disease, emphasizing the increase in the incidence of mild forms and especialy of forms with a subclinical symptomatology. Consistent arguments are brought lending support to the streptococcal etiology of the disease. Pathogenesis of acute rheumatic fever is also discussed including the relevant immunologic data and genetic factors. The authors present the clinical aspects of the disease, its manifestations, especially cardiac, with the sings and symptoms characteristic of affection of the endocardium, myocardium and pericardium. Rheumatic carditis are classified according to the degree of gravity. The therapy includes various aspects of the antiinfectious and antiinflamatory therapy, the latter being adapted to the clinical from of the disease and the degree of gravity. The prophylactic therapy refers to prevention of the first attack and the prevention of recurrences.
