Abstract

Immediate reconstruction after major oncologic maxillo­ facial excisions has crucial importance for
the patient’s quality of life and for early radiochemotherapy. Risks associated with long surgery and the patient’s concurrent disease are amplified by the risk of the free flap necrosis. That is why intra and perioperative medical therapy is most important for the outcome of surgery. In the Department of oro-maxillo-facial Surgery of the University Hospital of Stomatology “Prof. Dr. Dan Theodorescu” in Bucharest seven radial fascia-(oust)­ cutaneous free flap reconstruction for large maxillo-facial tumour excisions have been performed. Although most of the subjects were old, with moderate malnutrition, alcoholic and heavy smokers, none of them experienced serious complications. Here we expose the medication used to improve vascular motricity and permeability, blood rheology and clotting and the antibioprophylaxy. They have been used to increase the flap’s survival and integration chances.
