PARTICULARITIES IN THE MANAGEMENT OF SPLEEN INJURY IN THE CLINICAL EMERGENCY HOSPITAL FOR CHILDREN “GRIGOREALEXANDRESCU”, BUCHAREST

Abstract

The purpose of this study is to detect the key aspects of the spleen injury which can anticipate the evolution of the patient and the proper treatment. The study included all the patients admitted for blunt trauma of the spleen in a 3-year period. The data was collected retrospectively. The analyzed parameters were: age, type of trauma, degree of spleen injury, blood test results, ultrasound (US) examinations, findings at computed tomography ( CT) scans, type of treatment, duration of hospital admission, time in the intensive care unit( ICU), follow-up. CT scan was used in the evaluation of stabilized patients and the results were much more precise, especially in identifying those at risk for two-stage splenic rupture (p= 0.000014). There was no statistically significant difference regarding the duration of hospitalization according to the type of treatment. Furthermore, there was no correlation between the severity of the splenic injury and the duration of hospital admission. The conservative approach does not implicate additional costs and it can be applied in spleen injury up to grade IV if the patient’s condition allows it.
