Abstract
lliofemoral thrombophlebitis induced by oral contraceptives
The relationship between trombogenesis and estrogen and progestin contraceptives’ use is already evidenced clinical and experimental. To support this we present a case of acute ileofemural thromboflebitis which appeared after three month of treatment with oral contraceptives (Cilest and Marvelon). The onset of the disease was a severe edema of the left inferior limb extended until the thigh, together with cianosis, pain and functio lesa. The evolution during treatment with heparin and coumarins was difficult and unfavorable, so corticosteroids were added at the initial treatment. An improvment was seen after 40 days of treatment, without the complete release of the edema. The patient was released, but she came back 2 weeks afetr with symptoms of acute ileofemural thromboflebitis, even though oral contraceptives treatment was stopped 2 month ago. Venous angiography of the inferior limb shows the permeability of profound venis only in the distal half of the shank. Only the superficial venous system is permeable and realises the return of the blood. Anticoagulant and corticosteroids treatment was started again. This case is a warning, showing a severe acute thromboflebitis induced by oral anticoagulants in a young woman, without pathological history or other blood coagulation troubles.
